Patient Information
Dr. Fraga and our staff are dedicated to providing you with a pleasant visit and results that you’re proud to show off. Our administrative staff is ready to help you with questions about scheduling, financial policies, and insurance. Our goal is to make your entire visit and experience as simple and comfortable as possible.
Your initial appointment will consist of a comprehensive examination where Dr. Fraga will determine your dental concerns and goals. After gathering the necessary information, he will briefly discuss your diagnosis and treatment options. A formal treatment plan will be formulated and presented. We will make sure that you understand and are comfortable with treatment that we are recommending. After getting your approval, treatment can then be scheduled.
To prepare for your first appointment, please:
- Please download, complete and bring our Patient Registration Form.
- Prepare and bring a list of medications you are currently taking.
- Bring your insurance card (if applicable).
- X-Rays: If you have had recent x-rays taken (within six months), please have them emailed to our office by your prior dentist. If necessary, you can pick up a copy and bring it with you to your visit.
Please alert our staff if you have a medical condition that may be of concern prior to treatment (i.e. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are on any medication (i.e. heart medications, aspirin, anticoagulant therapy, etc.) or require medication prior to dental cleanings (i.e. antibiotics as a pre-medication).
IMPORTANT: A parent or guardian must accompany all patients under 18 at the initial exam and consult appointments.
Our office is open Monday – Thursday, 8:30am – 5:30pm, and we are closed for lunch from 1:30 – 2:30pm. Please give us a call and we will schedule your appointment as promptly as possible. Or click the link below and our scheduling coordinator will contact you!
We deliver the finest care at the most reasonable cost to our patients, and payment is due at the time service is rendered unless alternate arrangements have been made in advance. Please remember that you are fully responsible for all fees charged for our services, regardless of your insurance coverage. We do not base your diagnosis or treatment plan on your insurance coverage – we base it on your needs and desires. We take pride in the quality of care we offer our patients, and make every effort to have your dental visits with us as comfortable and stress-free as possible.
We will explain our service costs to you to avoid any confusion or misunderstandings, and so we can focus on your dental health. If you have any questions on your coverage, treatments, or costs, please ask. We are here to help you.
We work with all PPO insurance plans and are a Delta Premier Provider. We do not participate in any Health Management Organizations (HMO) or Medicare insurance plans. If you have any questions or concerns about your insurance coverage, please contact us and we would be happy to help you. As a courtesy we are happy to bill your insurance company for you.
Infection controls and universal precautions protect patients and staff alike. Everyone benefits from rigurous infection control – you, Dr. Fraga, and our entire team. The cornerstone of a good and safe dental practice is the element of trust. You should feel free to discuss this topic with Dr. Fraga or any of our staff and we will provide you with a straight forward and comprehensive answer.
Dr. Fraga and the entire team follow procedures recommended by several federal agencies: the Occupational Safety & Health Administration (OSHA), the Environmental Protection Agency (EPA) and the Centers of Disease Control (CDC). These procedures include:
- Use of disinfectant hand soap
- Single-use gloves, face masks, and eye protection
- Operatory surfaces are disinfected after each appointment
- Instruments are either sterilized prior to each use, or are disposable
We sterilize all reusable equipment, including dental hand pieces. We use an autoclave sterilizer, a device that kills bacteria and viruses by steam, heat, and pressure.
The best defense against disease is information. The more you know, the better equipped you are to make wise decisions about your health care. The more you know about our daily procedures and policies, the more comfortable you will feel.
